Our scheduling coordinator will get you scheduled for a convenient time.  She can also answer any questions you have regarding your insurance coverage.  Please make sure you have your insurance card handy, so she can take the appropriate steps to determine the benefits to which you are entitled.  This service is offered to you at no charge and no obligation.

Once your appointment is scheduled, save yourself some valuable time by printing out and completing our General History form prior to your initial office visit.  If the appointment is for a minor, you must also fill out the Consent to Treat a Minor form. You will need Adobe Acrobat Reader in order to view the forms.
